SMARTSTORAGE systems maintains audit trails of system data, including who added, edited, or deleted information. I may be asked to justify my use, addition, or modification of specific information contained in SMARTSTORAGE systems.
I understand that electronic signatures are legally binding and have the same meaning as handwritten signatures.
When my signature is required, my user ID and password together shall constitute an electronic signature.
My electronic signature establishes me as the signer of electronic documents.
My password will be valid for the length of time specified in by Medtronic (or administrator reset, whichever is shorter) from date of issuance.
Setting a new password for my user-ID (electronic signature) renews the terms of this agreement.
I agree to keep my user IDs and passwords secret and secure by taking reasonable security measures to prevent them from being lost or inappropriately acquired, modified or otherwise compromised, and to prevent unauthorized disclosure of, access to, or use of them, or of any media on which information about them are stored.
Passwords for the SMARTSTORAGE customer website shall not be visible on a screen, hardcopy, or any other output device.
Badges and User Names (IDs) shall be used to uniquely identify individual users.
I agree badges shall not be shared with, used by, or disclosed to others. Generic or group badges and shall not be used.
All user and system passwords, even temporary passwords set for new user accounts, are required to be at least eight characters in length; contain at least one uppercase letter; contain at least one lowercase letter and contain at least one number; cannot be more than 32 characters long, portions of associated account names should not be part of the password (e.g., user ID, log-in name).
Users are required to select a new password immediately after their initial logon.
When passwords are changed, previously used passwords cannot be re-used.
If a user suspects assigned login credentials have been compromised, they shall report it immediately to email@example.com or 1-888-869-4994 and change their password via the SMARTSTORAGE customer website.
If a user has lost their badge or their badge is defective, then contact firstname.lastname@example.org or 1-888-869-4994 immediately to have their existing badge deactivated.
If I suspect that my badge, user ID, or password has been stolen or inappropriately acquired, lost, used by an unauthorized party, or otherwise compromised, I will immediately notify email@example.com or 1-888-869-4994 and request that my password or badge be revoked.
At the end of my employment or need for access, I agree to return my badge and notify firstname.lastname@example.org or 1-888-869-4994 that access is no longer needed. Upon departure of personnel (mandatory or voluntary) or reassignment of responsibilities, the site must notify email@example.com or 1-888-869-4994 to remove access of individual.
Inappropriate access of the SMARTSTORAGE cabinet or the user website may result in the termination of access or removal of product.
Violating the SMARTSTORAGE operation manual or appropriate regulatory requirements may also result in termination of access or removal of product.
